Nplate (romiplostim) is a biological drug given by subcutaneous (under the skin) injection once a week to treat immune thrombocytopenia (low blood platelet counts). Nplate works to boost the body's production of platelets.
The lowest dose possible of Nplate should be used to achieve and maintain a platelet count of greater than or equal to 50 x 109/L.
Patients are initially started on a 1 mcg/kg dose of Nplate a week, which is then adjusted by increments of 1 mcg/kg a week to achieve the desired platelet count. The maximum dose of Nplate is 10 mcg/kg a week.
After just a single dose of Nplate platelet counts were found to increase over a 2-3 week period, according to the results of clinical studies.
In clinical studies, adult patients who responded to treatment with Nplate and achieved and maintained the desired platelet count did so on a median dose of 2-3 mcg/kg of Nplate a week.
Nplate should be discontinued after 4 weeks of treatment at the maximum dose of 10 mcg/kg a week if platelet counts have not increased to sufficient levels.
Nplate is also used as a one-off treatment to increase survival in patients with hematopoietic syndrome of acute radiation syndrome.
