Initial dose: 50 mcg orally once a day or 100 mcg orally every other day.
Maintenance dose: May increase by 20 mcg/dose at 4 week intervals.

No adjustments recommended, however, patients with renal insufficiency are more prone to the development of hypercalcemia and should by closely monitored.

Some patients may respond to as little as 20 mcg every other day. Most patients respond to 50 to 100 mcg once a day or 100 to 200 mcg every other day.
Calcifediol should be withheld if hypercalcemia develops.
Vitamin D and its metabolites are not dialyzable.
Patients should receive adequate calcium during calcifediol therapy. Serum calcium should be monitored once to twice a week during dosage titration, and approximately once a month after stabilization of the dosage.
