Usual Adult Dose for Attention Deficit Disorder

Initial dose: 30 mg orally once a day in the morning
Maintenance dose: 30 mg to 70 mg per day; may adjust dosage in increments of 10 mg or 20 mg at approximately weekly intervals.
Maximum dose: 70 mg per day

Comments: Prior to treatment, assess for the presence of cardiac disease and the risk of abuse.

Use: Attention Deficit Hyperactivity Disorder (ADHD)

Usual Adult Dose for Eating Disorder

Initial dose: 30 mg orally once a day in the morning
Maintenance dose: 50 mg to 70 mg per day; titrate dose in increments of 20 mg at approximately weekly intervals.
Maximum dose: 70 mg per day

Comments: Discontinue this drug if binge eating does not improve.

Use: Moderate to Severe Binge Eating Disorder

Usual Pediatric Dose for Attention Deficit Disorder

Age 6 to 17 Years:
Initial dose: 30 mg orally once a day in the morning
Maintenance dose: 30 mg to 70 mg per day; may adjust dosage in increments of 10 mg or 20 mg at approximately weekly intervals.
Maximum dose: 70 mg per day

Comments: Prior to treatment, assess for the presence of cardiac disease and the risk of abuse.

Use: Attention Deficit Hyperactivity Disorder (ADHD)

Renal Dose Adjustments

Severe renal impairment (GFR 15 to less than 30 mL/min/1.73 m2): Maximum dose is 50 mg per day.
End stage renal disease (GFR less than 15 mL/min/1.73 m2): Maximum dose is 30 mg per day.

Precautions

US BOXED WARNING: This drug has a high potential for abuse and dependence. Assess the risk of abuse prior to prescribing and monitor for signs of abuse and dependence while on therapy.

Safety and efficacy have not been established in patients younger than 6 years.

Consult WARNINGS section for additional precautions.

US Controlled Substance: C-II

Dialysis

This drug and dextroamphetamine are not dialyzable.

Other Comments

Administration Advice:

  • Take this drug with or without food.
  • Avoid taking doses in the afternoon due to potential for insomnia.
  • Swallow capsules whole or open capsules and mix entire contents with yogurt, water, or orange juice and consume entire mixture immediately.
  • Do not take anything less than one capsule per day, and do not divide single capsules.

Storage Requirements: Dispense in tight, light-resistant container.

General:
  • This drug is not indicated or recommended for weight loss.
  • The prolonged release of this drug should be considered in overdose cases.

Monitoring:
  • Cardiovascular: Blood pressure and heart rate (at dose adjustments and at least every 6 months); changes in fingers and toes
  • General: Signs of abuse/dependence; evaluation of treatment usefulness (at least yearly)
  • Other: Growth suppression (height, weight, appetite at least every 6 months)
  • Psychiatric: Aggressive behavior/hostility and psychotic and manic symptoms (at every visit, dose adjustments, and at least every 6 months)

Patient Advice: Avoid driving and other potentially dangerous activities such as operating machinery until you know how this drug affects you.
