Drug Detail:Suvorexant (Suvorexant [ soo-voe-rex-ant ])
Drug Class: Miscellaneous anxiolytics, sedatives and hypnotics
Usual Adult Dose for Insomnia
10 mg orally once a day at bedtime
Maximum dose: 20 mg once a day
Comments:
- Take within 30 minutes of going to bed, with at least 7 hours remaining before the planned time of awakening.
- If 10 mg is well-tolerated but not effective, the dose can be increased.
- Time to effect may be delayed if taken with or soon after a meal.
Use: Insomnia characterized by difficulties with sleep onset and/or sleep maintenance.
Renal Dose Adjustments
No adjustment recommended
Liver Dose Adjustments
Mild to moderate liver dysfunction (Child-Pugh up to 9): No adjustment recommended
Severe liver dysfunction: Not recommended
Dose Adjustments
This drug should be used at the lowest effective dose in order to achieve therapeutic efficacy and minimize adverse effects.
Special Populations:
- Exposure is increased in women and in obese patients. Particularly in obese women, the increased risk of adverse effects should be considered before increasing the dose.
CNS Depressants:
- When combined with other CNS depressant drugs, dosage adjustment of suvorexant and/or the other drug(s) may be necessary.
CYP450 3A4 inhibitors:
- If combined with a moderate inhibitor: 5 mg a day; generally not to exceed 10 mg a day
- If combined with a strong inhibitor: Not recommended
Precautions
Safety and efficacy have not been established in patients younger than 18 years.
