Usual Pediatric Dose for Pneumonia

Infants:

  • Under 2.5 kg: 900 units/kg/24 hours IM in 2 or 3 divided doses
  • Over 2.5 kg: 1000 units/kg/24 hours IM in 2 or 3 divided doses

Use: For the treatment of pneumonia and empyema due to staphylococci with confirmed susceptibility to this drug

Precautions

US BOXED WARNING:

  • NEPHROTOXICITY: Parenteral (IM) therapy with this drug may cause renal failure due to tubular and glomerular necrosis. Use of this drug should be restricted to infants with staphylococcal pneumonia and empyema due to organisms with confirmed susceptibility to this drug; use is recommended only if adequate laboratory facilities are available and constant monitoring of the patient is possible. Renal function should be carefully determined before and daily during therapy. To avoid renal toxicity, the recommended daily dose should not be exceeded and fluid intake and urinary output should be maintained at proper levels. This drug should be discontinued if renal toxicity occurs. Coadministration with other nephrotoxic agents (especially streptomycin, kanamycin, polymyxin B, colistimethate [colistin], neomycin) should be avoided.

Administration advice:

  • Administer IM only.
  • Administer in the upper outer quadrant of the buttocks; alternate right and left and avoid multiple injection in the same region (due to transient pain after injection).
  • Maintain adequate fluid intake orally or, if required, parenterally.

Storage requirements:
  • Unreconstituted vial: Store in refrigerator at 2C to 8C (36F to 46F).
  • Reconstituted solution: Stable for 1 week when stored in refrigerator (2C to 8C [36F to 46F]).

Reconstitution/preparation techniques:
  • The manufacturer product information should be consulted.
  • Diluents containing parabens should not be used for reconstitution.

Monitoring:
  • Renal: Renal function (before and daily during therapy)

Patient advice:
  • Consult physician without delay if watery and bloody stools (with or without stomach cramps and fever) develop.
