Use of this drug should be restricted to infants with staphylococcal pneumonia and empyema due to organisms with confirmed susceptibility to this drug; use is recommended only if adequate laboratory facilities are available and constant monitoring of the patient is possible. Renal function should be carefully determined before and daily during therapy. To avoid renal toxicity, the recommended daily dose should not be exceeded and fluid intake and urinary output should be maintained at proper levels. This drug should be discontinued if renal toxicity occurs. Coadministration with other nephrotoxic agents (especially streptomycin, kanamycin, polymyxin B, colistimethate [colistin], neomycin) should be avoided.
