Along with its needed effects, varenicline may cause some unwanted effects. Although not all of these side effects may occur, if they do occur they may need medical attention.

- Anxiety
- black, tarry stools
- bloating or swelling of the face, arms, hands, lower legs, or feet
- bloody stools
- blurred vision
- burning feeling in the chest or stomach
- chest pain or discomfort

- deep or fast breathing with dizziness
